New Patient Registration
We accept new registrations, both permanent and temporary, for patients that are resident with the practice area. Please apply in person at Reception for the registration forms.
You will be asked to provide:
- proof of address (e.g. utility bill)
- proof of ID (e.g. passport, driving licence)
- name and address of previous GP
- NHS number (if known)
Please ensure that you complete your forms clearly, otherwise this may delay your registration process. Please allow 48 hours for completion of this process.
If you have recently moved to the UK and wish to register with the practice, you will also require photo ID, e.g. Passport.
Repeat Medications for New Patients
Please note that if you are currently on repeat medications from your old surgery, you will be required to see one of our GPs Clarence Medical Centre before any repeat prescription requests can be made or issued.
New Baby Registration
New babies must be registered at the surgery prior to their six week check up or before any vaccinations being carried out. Please supply a copy of their full birth certificate and a copy of the discharge notification, if available.
Practice Area
Extends up to the Thames, Home Park, King’s Road, Winkfield Road and includes St Leonards Hill and Oakley Green only as far as the 01753 telephone code area. Excludes the Great Park and Old Windsor.
